Thats a the advanced bbcodebox v5.0.0 modded to do youtube and google videos. If I remember correctly I think I also tweaked it to fit in with the theme we use here. It looks like these guys have one packaged up already that does the videos so it might be worth a look. http://www.clan-themes.co.uk/downloads.html
The videos are sort of a bitch to get going but its worth it

There is also a pretty cool quick reply hack for the forums that goes along well with the advanced bbcode box. I just need to remember how the hell I did it. LOL
